2024 Industrial Safety Contest Entry Form

This award recognizes trucking companies with the best ratio of hours worked relative to time lost due to industrial accidents. Winners are selected in various size categories.

2.  Obtain and upload your OSHA logs
3.  Complete the required information below


RREPORTING REQUIREMENTS FOR THE INDUSTRIAL SAFETY CONTEST ARE AS FOLLOWS:

- Contestants shall be in good standing with North Carolina Trucking Association and must possess a USDOT number.
- All employees are to be counted, not just those in NC.
- Total number of employees and employee-hours worked must match those reported on the OSHA logs.
- Please note number of cases - not number of lost workdays.
- Frequency rate is determined by multiplying the total number of lost workday cases by 200,000 dividing by the total numbers of man-hours worked.
- Failure to consent to an audit or rendering of records will therefore make the contestant ineligible for any award for the contest year.

All 1st place winners in any category and those with Zero Accidents will receive awards.

Check one.  Contestants shall be in good standing with the NCTA and muss possess a USDOT number.  All divisions must have a minimum of three participants before the division will be judged in the contest.  Contestants will be notified if their division has less than three entries.
